Last time I played against trail, the aura showed me the gen to do in a 3 gen situation. Which led to a loss for the killer.
That tier is also called weak/niche, so it makes sense.

Why do I see Doctor listed so low on tier lists when he seems to be one of the easiest and somewhat pretty powerful killers, in the games I've played with him in pubs and customs with friends he's super easy to use and win with

Very hard to play to a high level, terror radius builds often make it very easy to W strat, he needs add-ons to be good, some of his add-ons suck, his shock basically needs the green delay for anyone with half a brain or to make the majority of loops actually benefit doctor, his static reveal is quite a slow cooldown and also relies heavily on terror radius, you only really get true negatives in T3

You need to use the shock therapy to shock them so they can't vault or drop pallets but keep in mind a delay

There's next to no benefit from using them all except very select scenarios and the speed debuff just makes it even worse
His best add-on build is easily purple chilli and green speed, though his replenish charge yellow and any chilli is good too
Can make use of iri flesh but it's less consistent
His aura when revving is decent enough too for loops
