Yes. If an Adreno shot hast complete and you other two team members are knocked or dead, the whole team is killed off. 1 member has to be in game and on their feet.
Keep in mind the Auto-inject option is not wanting to activate sometimes right now. I've had to come back to my teammate and hold revive until they get up.

Only thing I can think of technically the adreno shot wasn't yours
Like if you were the last of your friends to go down and you have one in the bottom left under your health bar gives you option to use it.
But since your teammate was last giving his adreno shot to you make him lose his and the adreno shot was "used" and wasn't transfer to your backpack as you don't have the symbol bottom left of the screen before he dies. Therefore you all die which is dumb but hey this game is what it is

Yes, that's what I thought. I just want to know if it's intencional or a bug from the game... it's not a usually situation to see if always happens
