Not sure if this happens due to axiom, but I'm pretty sure seeing as this bug showed up earlier in here.

• OpenJDK 64-Bit Server VM, version 21.0.3
• Fabric 0.16.10 on 1.21.4 using 5 mods

try with only axiom and fabricApi installed and add back your mods one by one
doesnt look like an axiom related crash

Btw other mods (xaeros mini map) will not work with flashback or cause issues
