you have just (likely) messed up your Hammer settings/configuration

To expand on Squids answer that is the default icon that you get if you do not have the right FGD's set. FGD's tell hammer what entities exist. If you put in an entity that hammer doesn't know about (e.g. you decompiled a CSS map and opened it in HL2 hammer and it doesn't understand what info_player_terrorist is) you get that icon
