Specific spell location in Root.wad -> Spells/Fight Specific/HousingGauntlet/SIT Gauntlet/Blood Moon - Ubah.xml
The spell description is +#1:$eA$% $Outgoing_image$ #2:$dT_image$#3:$Damage_image$
I think because the m_effectList value at eA is a negative value it renders with the negative symbol but the description defaults to having a + at the start of the description so it renders like +-25% $Outgoing_image$

Spells that players can't see generally don't have to have super accurate spell descriptions
