You can approach it via mechanics or art.
In terms of mechanics, try to think of extensions of things that exist or finding new niches. Jokers generally have a trigger condition and a reward. Pick one, then try to think of the other and the consequences thereof

For example, taking Bootstraps, you could make a Joker conditional on having money, but rewarding with more money might be too bland
Although we do have To The Moon
You could grant a reward you could get with money, like Jokers, or a reward you can’t get with money, like Boss Rerolls (at least not easily)

Or maybe a combination of both—maybe you create a bunch of Negative Jokers, but lose them when you lose the Joker. You could get them with money, but not easily, but it also locks you from spending the money anyways

I think doing it flavor first is harder
What you can try to do is split it 50/50
Think of something condition or effect and see if you can shoehorn it into some theme

Ok but what can work is thinking of a theme, trying to find out a relevant mechanic, then coming up with multiple effects within the theme
Like, Space (theme) is related to Planets, Poker Hands, and Levels
One of my themes was Fish, and I knew I wanted Fish to reward Flush Five. Rewarding Secret Hands is similar. To avoid mentioning Secret Hands, one way to approach it was to reward playing different hands or reward playing higher “tiered” hands

But, as an example, I have a Joker which I came up with before Balatro released. At the time, my idea for it was like a Luchador, but, well, since Luchador already exists, it doesn’t have a set flavor yet.
It grants bonus money in exchange of increasing the Blind Size. To me, this gives an idea of overcoming a big challenge. So I’m leaning towards climbing a mountain or a staircase as a theme
