When you enter the blind, a special card will enter your deck, you need to either play or discard it to destroy it to be able to score hands, to make up for this, the required score isn't that large, probably 1x or 0.5x blind requirement

As currently it's just a hidden rng check that you only figure out at the end whether it'll kill you

But there's no deck manipulation during a blind in Balatro, so if the card spawns at the bottom of your deck, you're dead, unless you've happened to run into enough card destruction to thin your deck, or happen to have found a very specific set of jokers or vouchers

I have that blind basically as the opposite in my mod as a Joker (except you can't see which card is the marked card), and it's conceptually a fun design, but not very fun to play with. I think finding a card might be the issue there, as Balatro's got no way to involve skill or strategy in that regard

Not entirely. Talisman has a lot of issues, and if it's something you're interested in, an alternative would probably be welcomed. Or you could try and see if you'd be able to optimize or bugfix Talisman, as it's not got any active devs besides people doing some maintenance atm, I think
