I don't think that's true, the problem does not state that you have to kill anyone, or that you have to stop her yourself. Problem just asks what would you do, given the option to kill 20 people to stop her, and I don't think deciding who lives and who dies is that much different from what she intends to do. When she kills 50 people, it's not your fault, but when you intentionally crash the trolleys, I see it as you were the killer of 20 people.
In the end, I don't think there is a correct solution cuz both options suck.

i think local thunk might hate the number four

think about it, the sinful jokers have been nerfed to +3 instead of +4
meanwhile, smiley face has been buffed to +5
on top of this, even gros michel has had its probability of going extinct changed to a 1 in 6.
and while localthunk most likely couldnt get away with completely removing 4oak from the game, he has done his best to try, changing clever joker and mad joker to proc on two pair rather than 4oak.
whats more, thunk even tried to eliminate the +4 chips scaling on square joker. since with the fact it also triggers on four card hands, thunk thought it so repulsive that he had to change the scaling. unfortunately for thunk, his plot was foiled when the fanbase responded with huge backlash, claiming the +5 scaling was too overpowered.
watch out gamers, in the next patch, localthunk might be coming for your problematic faves, such as astronaut, the family, walkie talkie, hack, even steven, and more. for all we know, the future of balatro may be one in which all cards have 3 or 5 sides.

my evidence for this, lies in the fact that Balatro originated as a way to play Big 2 digitally

alternatively it might be due to the fact that the game having so many +4s makes the math more predictable, i think thunk might be just making balancing changes to keep us on our toes, just so that we can feel something, anything

not devolve into mindlessly believing "balatro science"

switching the balance from consistent +4s to +3s and +5s invites chaos into the game's meta
