One of the SimPlayers died during combat but didn't revive when fight was over. I'm not sure if it would help to reproduce it but they might have died to poison?

Here's another case of no one reviving after combat. I trained the mobs into NPC guards but the SPs stayed dead. I had to zone to get them to revive.

Kind of have to stand right on top of them to get them up
I'm going to suspect this has something to do with mods, your no faction hit and fails to revive are both related to combat ending which it seems to not be
