its all about years of skill ^^'
mostly use refereces as mutch as possible...
If fancy you use a doll tool (like Frosty showed) so you basiclly are freed from limited angels and stuff 🙂
shadows are well alot of thinking, where is the light source? and where it would cast shadows / and highlighs

but for references you can use them as you please, i remeber a art piece i did, where i mashed up two screenshots together to get the desired highlighs of features of a char.
