These events happen if you use blunt force or If you drop him on railings. When it happens Stanley reacts to it by holding his crotch and Stanley dealing with the pain for at least a minute. In a much more extreme case, a right amount of force (or using a gun and shooting Stanley in the crotch) causes the "testes" to rupture, and send him to a seizure, and possibly kill Stanley.
