You can have this as a setting also and turn it on and off

at one point will someone let the game run out for 20 minutes just to be like "ah yeah... he does pass out from thirst I guess"
and then never do it again?
does it really bring any value to the game?
the only real result I can think with this is... delaying the release or a post release update to add that.

I think you should just make it a different game. Make a game like sims where you take care of him. Also no guns as that is too violent
