Having women 40+ years of age having more children after you move their older children out of a house into their own really breaks immersion for me. Maybe a max age could be put insetting's if others want to keep it as it is. For a medieval style game kind of doesn't fit to me.

As someone who is close to that age myself, currently pregnant with my 4th and my two oldest children in high school…. I don’t think 40 is an unrealistic age at all. I can see your point though. But maybe at a higher age would feel more “realistic”
for medieval times, having a child at 40... The lifespan wasn't the same as the lifespan we have now in 2023
actually, it somewhat was: what messes up with the statistics is Infant mortality.
Basically, if you made it to 5yo, then you would be able to live to 60, 70, even 80 or more with a bit of luck. But the hard part was surviving infancy.
My knowledges of medieval history aren't agreeing to everything you said. High mortality at early ages, yes. But medicine wasn't developed as much as it is now so 70+ I don't think so

it wasn't common but it did happen
and medieval times weren't that bad, medecine-wise (it worsened a bit with renaissance, actually)
Just to ad .... Wars also took down the average life span numbers a fair bit.
