I'm thinking of other potential blunders. A correlary of 2 is that it's never doing anything in the background or while you're waiting on it. It doesn't recognize time has passed at all. For one more, maybe

You (or the AI) used the wrong tool for the job.

That would be it trying to create images that it can't reasonably do and weren't asked for, trying to modify file types it can't handle well, trying to run Python scripts that fail on execution, using a web search to get bad results (this happens all the time – super important people know that the model it uses to handle searches is WAY different and their results change dramatically if you just tell it not to do this), using or overestimating the use of a GPT, etc.

Another one would be

Your context is polluted.

That's when you have weird memories, start with a bad GPT or bad system prompt, CIs or memories. A ton of times it'll stop completely l when you just try a new, vanilla incognito session.
