How proprietary, difficult to replicate or "special" can prompts be?

I am wondering, because these services are starting to come up, where the back end, is chatgpt4 (constrained by a prompt), and the front end is an interface like Dr. Gupta
How hard would it be for someone else to create a similar "doctor"
Would one doctor be superior to another?
Or is a prompt as simple as "You are now a doctor" - as good as any other prompt?

They try to make them propriatary but they usually leak
And running the same prompt gives you the sameish AI pretty much
