Unreal is very well optimized. Honestly most optimization issues UE games have are third party plugins or new systems the dev developed and either did not or could not optimize themselves.

There are allot of people who take the easy route and blame the engine.

The trainer mod was developed for an older build of the game which also used an older version of UE engine. The mod tools used to develop the trainer have not yet been updated to the latest version of UE so the trainer developer cannot update it. He would have to rebuild the trainer for the new demo itself.
