Alright, so upon further testing, orange items seem to indeed be a result of looting a "unique" item from the abyss that you already have at least one of and insured. Upon arriving back to the tavern from the abyss with one of these items, it will be orange and the insured yellow item you already had in your inventory will also turn orange, and it's likely that any future instance of that item you see may be orange, especially if it's from the abyss, from what I can tell

It doesn't have to be from the abyss though, finding the item in the abyss is just the catalyst to turning the items orange

For example, I just looted this unique axe from the abyss for the first time on this new character on an old save, prior to this, the axe I had in my inventory was just insured yellow. But after coming back from the abyss with one, the one from the abyss is orange and it also turned the one I already had in my inventory orange.

But, as the video shows, there is one of these axes for sale at the merchant and it is colored orange. I did not sell this axe, I haven't sold anything to the merchant for a few days now, and I don't think I ever sell unique weapons like that, just stuff like goblets/chalices/stolen armor/weapons(modular) - I know the look of this axe and wouldn't sell it
