when you train with a heavy weapon and run your stam down over and over you build str, I built a max weaight poleaxe and use it half sword stance, hit f and swing away and try not to knock myself out to much, I like to get foggy and then run and lap or 2, this helps your speed awell imo, keep an eye on your weight and keep it above 90kg

It was tied to burning your stamina. Fastest way to level was exerting yourself to the point of passing out, which you could do after a fight by punching walls or a downed enemy until you black out and fall over. Easy 2kg of muscle mass per match. Still a grind, but doable.
As of the current patch I am completely unable to gain muscle.
