Pretty sure the gore settings just apply to you and the NPC, so you can lop their head off with a little nick, but the same goes for you.

the main and most useful application that i see for this is to greatly enhance slashing attacks

Still difficult to use, it eats stamina ASF and any push from the enemy will knock you off your feet if you are out of breath and do not break the distance
