Does this happen after booster seperation?

I was going for low mun orbit
It looks like your command module is upside down, the issue could possubly be that the game thinks your rocket is facing the ground. Try using the "Reverse" control point option in the pod

I recommend putting a probe core on the mpl segment and controlling it from there, that should fix it and gives you the bonus of maintaining control of it without a crew
also, dont forget parachutes on the return capsule. I cant tell if you have them or not from the screenshots, but just in case
Honestly, I just got rid of the boosters and it's fine now. I think my center of mass was too low. And thanks for the reminder regarding parachutes, that would have been an awful surprise lol.

also, you want your CoM to be high for a rocket
CoL should be below the CoM, and the farther they are the more stable the rocket
With probes, am I still able to maneuver and use all the SAS fine?

depends on the probe core what SAS modes you can use, but yes you can do all that as long as you have a connection to kerbin
the antennas on it already will be fine for the mun, but you may lose signal while crossing the far side of the mun
