I was getting this... Reddit suggested to manually decouple from the parts manager. Seems to work (at least for the moment). Something to do with staging not working, rather than the decouplers

I moved decouplers into their own stages for now. Easier to control.

Was it attached to the attachment-node of a docking port? There's a known bug with other parts attaching there.

I also had this happen and no matter what I did, stage separation would not work but manual part manger did (sometimes)

I had the same issue multiple times. Even manual activation fails after it has been staged. I am forced to revert to VAB and rebuild the decoupler, engine and fuel tank above the broken decopler to fix it.
