I have re-run all and triple checked every node but still stuck there, I attached the save file.
There is still a chance I might be missing something obvious but couldnt figure it out

double check the debris field nodes. they get a mark when entered the first time but don't count as finished for the achievement until their cost is 0

ya for some reason G5 D2A I think it was is often not gotten.

So I went back, nad my G5 D2B was not at 0, so I then got it down to zero and did a full run of hte galaxy getting everything all over again, and Im still at 364/365. Checked GF nodes, and all other debris field nodes and I can't find what Im missing

I had this same issue, can confirm that one of my previous galaxy's A nodes was not completed (I guess I never needed to complete that node for all the artifacts!)

I had found the galaxy 2 I believe A node pretty early on, I went on a systematic galaxy by galaxy completely full run and finally got it here

They do have to be reduced to zero cost, yes.
