With my experience, there is some gap between 1st savage and extreme, moderate amount of gap between 2nd and 3rd, and 4th is really difficult

Around 30% HP left in extreme contents is not good. Usually the first 2 extremes of the expansion is really easy compared to the usual extreme contents

Typically Job Quests are contained within their expansions separately. If you have not reached the next expansion in , you need to continue the main story quests until you reach the next expansion, denoted by giant letters naming said expansion.

Entering an encounter before clearing the previous floor(s) will relinquish all rewards from previously uncleared floors. It's more of a progressive gear shift constantly. The jump you're seeing from 50 to 90 to 130 is a combination of end-of-expansion artifact gear superceding leveling gear, and then the highest-tier raiding gear becoming the default Poetics tomestone gear once the new expansion is out.
