Also btw, while I don't have any insider info on the XIV Servers infrastructure, I can surely say that they probably have "provisioned" servers so whenver it detects too much people it starts getting up new instances, that takes a while tho, so bear a little with the errors and it will sooner or later work again.

It works differently... so cod works in small "instances", you have up to lets say 20 people together in a "room", those rooms get "created/destroyed" on demand. This means that the server can expand whenever more people get in.

In a mmo server structure, you also have instances, but those are much much much bigger, and the cpu/ram usage due to that is way bigger. There's also the database component to the infrastructure, which needs to handle many many many more actions per second than Cod does. Most information in cod is local-based, and only some information gets recorded (kills, bullets).
