so you could start as a marauder (which is tank) and eventually when you enter ishgard, unlock astrogolian
the requirements for expac jobs are 1. own that expansion, 2. meet the level requirement, and 3. in heavensward jobs (ast, machinist and dark knight) have access to ishgard
And for the most part, most groups don't care about specific healer

I would group dungeons, trials, raids, alliance raids into normal content. As everyone else stated, extreme trials are next in difficulty, followed by savage, then ultimate being the most difficult. Within those tiers, anything being closer to current max level will be more difficult than past content of similar style.
