You need to achieve a "yaku", and also complete the rest of the hand to win. One of the most common yaku is a hand with no 1s, 9s, dragons, or winds, for example. Another would be having a triplet of the same color dragons, or a triplet of winds that match the round wind, or your wind.

Items in the glamour dresser can be reused infinitely for glamour and can be placed in glamour plates which are basically full sets of glamour you can apply with one click. You can also assign glamour plates to gearsets so the play gets applied automatically when you switch to that job
I use glamour plates so I have a decent different look for every healer even though they're all using the same gear
