items dont scale with your character level, character level just determines what gear you can equip + new skills. so, to your question, yes, i130 on a lvl 50 would be the same as i130 on a lvl 58.

Scaling only works downwards, if you're wearing lv60 gear in a synced lv50 dungeon your ilvl will go down to whatever is the highest ilvl for that lvl (iirc). But wearing lv50 gear in a lv60 dungeon is still the same.

You can just ignore positional requirements for such enemies
Usually used for enemies where it’s either impossible to reach flank/rear because they’re off the arena, or when it’s otherwise not viable to do so e.g. in PotD where you might not have a tank to point enemies away from you
