Generally when you finish the expansion story and have done the normal version of said trial @leaden ginkgo
After doing the extreme version unlock quest
The first 3 ARR trials are a bit different since you have to unlock the hard version first, do those, then unlock the extremes
Those 3 are the only ones with 3 difficulties

When you start getting into The Hunt you can buy Aetheryte tickets which give a free teleport for one ticket. If you keep up with the hunts and use them for that you should rarely ever need to pay.
