In that case, for some reason if you buy the upcoming expansion on pre-order, you don't get the previous stuff until the day the expansion. The only thing I can say is each key has to be put in individually into the mogstation. I'm not sure what else I can say to help, but I have 4 keys and each one had to be put in separately. If you have shadowbringers separately from endwalker, you should be able to play past heavensward without waiting for endwalker release.

Correctish, the penalty also applies if you fail to commence duty from Duty Finder three times. And then every time after that, a full penalty. (Resets faily)
This penalty does not apply if someone else leaves FIRST. If you leave second or third etc, you get a free pass. But if you leave the duty first before its done...

Often, one or two (or more) of them will have something available as a Bonus to Deliver.
For example this week for me, Adkiragh and Kurenai had a bonus for DoH and DoL respectively - but last week, I did Zhloe and Ehll Tou because they had the bonuses instead. Even if from a previous expac, a Custom with a bonus is still better

you can level them all together, they rely on each other for crafted materials, CUL is the exception, you can max that first if you want for custom deliveries and endgame scrips
