I completed that first few steps of this Fable, I have Witch Verm in my Nighthold. However, when it comes to using the Sap Tap to recover the sap to progress the quest, I am unable to do so because I am missing the Sap Tap. I never received this quest items.

I have logged out and spoken with Witch Verm, multiple times. I returned to the location we met the rat and Witch Verm as well as the cave with the initial encounter with the bear. None of these steps have helped progress the quest or given me the Sap Tap.

I am not sure if this is because of the quest not progressing, but I am no longer receiving faction bounties in the open world either. So I am unable to progress my faction reputation as well.

You are blocked in collect the sap tap in the quest

Yes, I have not been able to progress past that point.
