While exploring the Undercity of Dermak dungeons, occasionally these super blurred boxes will appear. Destroying another nearby box seems to fix the blurred textures, but I often find them in either secret rooms that require lever-pulling to get into, or in the final reward rooms of these dungeons, with usually about one of these boxes appearing per dungeon.
