Mostly based on gear/valuables you pick up so if you get a full inventory of legendary trinkets you will get an insane amount of AP but if you have no trinkets you will have no ap you get less ap for LOOTED gear but less than most treasures killing bosses/going down to hell or abyss will also give you ap but you must live in order to gain ap

you lose ap depending on rank. if you look at it theres a requirement of ap you lose per queue on your rank. if you die you lose those points as well its how demotion works in this game

if you see demon mouth, demon stairs, blood river, blood bridge, goblin maze, spider lair, demon gate, or blood prison--

I see what you mean, All the other ones say "any" rarity. I might have been mistaken, but Im am fairly confident that I turned in two purple bangels from a HR run to finish that quest. I assumed the Rare rarity was a minimum.
