Kanata has suffered from Meniere's Disease her entire life. It is a chronic condition that affects balance and in many people, including Kanata eventually causes partial or total deafness. At debut she was already losing hearing in one ear and during her time in Hololive she lost it entirely and her second ear started to go.
Kanata's character designer Oshio said it best "So it's come at last..." as fans of Kanata knew it was always possible and graduation due to the disease was a "when", not "if."
