Bug Report: App action approval dialogs fail to submit correctly until after page reload
`Steps to Reproduce`
- In ChatGPT web, trigger any app/tool action that requires approval, such as GitHub or Gmail.
- Click the approval button in the request dialog.
- Observe that the dialog closes, but the assistant stays on “Thinking…” and the requested action does not complete.
- Reload the page.
- The same approval dialog appears again.
- Click the approval button again.
- Observe that the requested action now succeeds.
- Trigger another app action and observe the same behavior repeats.
`Expected Result`
Clicking the approval button once should submit the approval, complete the requested app action, and allow the assistant’s turn to continue.
`Actual Result`
The first approval click dismisses the dialog but does not complete the requested action. The assistant remains stuck on “Thinking…”. After reloading the page, the same approval dialog appears again, and approving it a second time completes the action successfully. This repeats for each new app action request.
