Open Codex on macOS.
Make sure Codex.app and Codex Computer Use.app are enabled in Accessibility and Screen & System Audio Recording.
In Codex, use the Computer Use plugin to attach to an app like Google Chrome or Finder.
Call get_app_state for that app.
Watch for an approval prompt in Codex.
Codex Computer Use should show an app-approval prompt for the target app, or attach directly if already approved.
After approval, Codex should be able to inspect and interact with the app window.
No visible approval prompt appears in Codex.
The request fails immediately with:
Computer Use approval denied via MCP elicitation for app 'com.google.Chrome'.
The same happens for Finder, so it is not Chrome-specific.
Local helper telemetry records computer_use_mcp_app_approval_requested followed immediately by computer_use_mcp_app_approval_resolved with computer_use_mcp_approval_result = declined.
