Bug Report: Codex app does not refresh available MCP servers in existing conversations
`Steps to Reproduce`
  • Install an MCP server from the skills library (ex. Figma)
  • Ask the agent to fetch using this MCP server
  • Note that the agent claims the MCP server is unavailable
`Expected Result`

The MCP server list should be updated for my next prompt with the agent

`Actual Result`

The MCP server list is not updated
