Bug Report: Codex Extension in VSC Message Disappear

`Steps to Reproduce`

Not sure how to reproduce, but sometimes when sending a message it just gets sent to the void instead of going to that queue where you can click "steer". Not very rarely it just disappears. I've made an habit of ctrl c'ing my prompts before sending due to this thing that happens sometimes.

prompt should go in queue

instead disappears completely.
