Bug Report: “Scan Tools MCP Server” error shown when submitting form
`Steps to Reproduce`
  1. Go to https://platform.openai.com and create a new app.
  2. Fill in all the required fields in the app configuration form.
  3. Configure a valid MCP server (reachable and properly set up).
  4. Click on “Scan Tools”.

`Expected Result`

The form should submit successfully and display available tools

`Actual Result`

The submission fails and returns an error referencing “Scan Tools MCP Server.” The form is not submitted and no confirmation is shown.
