Bug Report: Image generation is stuck

`Steps to Reproduce`

I ask an image to be generated after I have already generated one.

A new image

I get the same image over and over again, no matter what I say or do.

Could you tell how this happens with you? More we get, better.

I reproduced this behavior across both 5.2 and 4o models. I usually issue my requests with something like:

"Create an image:"
Followed by the prompt.

I tested other variations such as selecting the image tool, trying both web and iOS app, and even changing the command to instruct a new image, a variation. It looks like some form of caching or replaying the same seed.

A new chat thread will create a new image for me. Also if I pass in what doesn't look like a traditional "image prompt" then that works as well, but that's likely because the ChatGPT model is building the prompt on the fly and differences in prompts create a new image.
