Bug Report: Uploaded images dont work and "My Media" is broken
`Steps to Reproduce`

All uploaded images from before october just broke and are imposible to use. Since the beginning of Nevember, images created dissapear from "My Media" and failures just wont go away, even if deleted.

`Expected Result`

I expect that previously uploaded images to be there, and from "My Media" to show the images i created and the failures that i delete to remain deleted.

`Actual Result`

All uploaded images from before october just broke and are imposible to use. Since the beginning of Nevember, images created dissapear from "My Media" and failures just wont go away, even if deleted.
