Bug Report: My ChatGPT App Disappeared

`Steps to Reproduce`

Go to Settings > Apps & Connectors > Advanced settings and turn on Developer Mode.
Go back to Apps & Connectors and create a new App. Login if there is authentication. Once complete, see the "success" message at the top of the screen.
Start a new chat. Use the / command to reference your App by name.
Query the App.

`Expected Result`

The App server is successfully called and returns, showing a custom UI.

`Actual Result`

ChatGPT correctly identifies the App and forms a Tool call for it. The call is never made and ChatGPT responds "I can’t place that trade because the trading tool is currently disabled."

`Environment`

ChatGPT Web, Mac OS 15.6, Safari browser 18.6

Previous chat can’t load the widget and adding a new connect load without result - no error code on the front only error 404 find on the console

Plus 1 to this. And connectors has been flaky all week, noticed several times it changed overnight. This is a really poor devex.
