Bug Report: File memory persists and duplicate chats appear despite memory off
`Steps to Reproduce`

Ensure memory is turned off in Settings.

Open ChatGPT desktop app (Windows 11, version 1.2025.258).

Start a /study chat and upload a file.

Delete the chat.

Go to Settings → Delete all chats.

Repair or reset the app, then relogin.

Start a new /study chat.

Observe that ChatGPT still recalls the previously uploaded file.

Also observe multiple empty chats (3–10) created automatically when using / commands.

`Expected Result`

After deleting all chats and resetting the app, ChatGPT should not retain any file memory or previous session context. Creating a new chat with a / command should create exactly one new chat, not multiple.

`Actual Result`

ChatGPT recalls the previously uploaded file even after full deletion and reset, and spawns several empty chats on / command use.
