Bug Report: GPT 5 being suddenly incompetent and stupid
`Steps to Reproduce`

Give detailed instructions on how to describe an image, and supply the image.

`Expected Result`

Instructions followed and image described accurately.

`Actual Result`

One or another or multiple rules/instructions are just ignored. Image described inaccurately, sometimes wildly so. After pointing out the mistake and asking for it to be corrected, it complies but quite often finds some other way to screw up instead.
