Bug Report: GPT5 and 4o fail to read files but pretend to.
`Steps to Reproduce`

I have recently been asking GPT to help me review and summarize papers. Some example chat history is listed: https://chatgpt.com/share/68a781e6-f450-8008-bc0a-e3c050f6e9f4

`Expected Result`

True information about the documents.

`Actual Result`

After several attempts, it would just pretend to read the document and show notifications like “reading document”, BUT! In fact it has read nothing! And it pretends to give a totally fictional summary including fake title, fake authors, fake techniques and results. It keeps happening no matter on GPT5 and GPT4o.
