“GPT-5 Fast isn’t reading the documents in the project (knowledge) before answering, even though my custom instructions specify it should do so for every response. If I use GPT-5 Auto, it sometimes takes time to think and not faster. Previously, GPT-4o worked fine for this, but now even 4o doesn’t consistently follow the custom instructions.”

abilitiy to answer from the source data of knowledge files for every query (that too i have explicityly given this custom instruction)

it is giving result without checking source files.
