Bug Report: Unrelevant Responses

`Steps to Reproduce`

I prompt gpt-5 with clear instructions to write me a document. Gpt-5 ends up responding with unrelevant things from past conversations or just totally ignores me. Been trying for 3 days now..

`Expected Result`

He actually writes me the document that I clearly instruct him to do.

`Actual Result`

He started writing about something from another conversation. Continued something we had already finished. He jumped right into the middle of a discussion from another conversation that was already past.

Latest android 15 and ChatGPT app. Plus Sub.

Now when I went back in the app to take some pics, the conversation had an error like "something went wrong" and all the data from that conversation is gone.

My memory is no where near full. The conversation was started like 1h ago.

I want my gpt-4.1 back.
I am paying for chatgpt plus but haven't been able to use the app since I got the update with gpt-5..

I also wanna report that 2 times today I have sent 2 big prompts to GPT-5 and lost the prompts. I wrote it and sent it, GPT-5 started responding to something totally different and didn't even get my messages and they were totally gone. I even tried asking if gpt-5 could send me the last message I sent it and it was not the prompt i just sent.
It was totally lost.. This should NOT happen.. Since gpt-5 release, ChatGPT has been so bad.. I am paying for Plus and its not even working.. Such a waste..

This keeps happening aswell...
