Bug Report: The "usage policy violation" message or error appears only when using o3.
`Steps to Reproduce`

Select a model using o3 and ask a question.

`Expected Result`

As with other models like o4-mini, a response should be received for the question.

`Actual Result`

Inference is performed, but the message "Your request was flagged as potentially violating our usage policy. Please try again with a different prompt." or the "getNodeByIdOrMessageId" error appears. In the same chat, after the "getNodeByIdOrMessageId" error appears, reopening the chat may change the message to a "usage policy" error.

ChatGPT web version, Mac app version, iPhone app version.
