Bug Report: "Failed to Comment" "Failed to Generate"
`Steps to Reproduce`
In a project, attempting to tune project instructions. The model is using user custom instructions. I'm trying to get it to ignore those instructions and only use those for the project. When asked "tell me about this project" it always includes user instructions. I respond "Your response doesn't align with your system instructions that are specific to this ChatGPT project. Please modify those instructions to strengthen the point that we do not want details to be used from the user instructions."
`Expected Result`
Expected a revised project/system prompt. This sort of tuning works outside of projects.

Two errors: Failed to Comment, and Failed to Generate
... but then it responds anyway.
The response is sometimes pretty good but it also tries to create a Canvas document for the text, and then includes commentary in that document. It's confused about content versus commentary when a decision is triggered to create a document. After the document, it does show the option: "Answer in chat instead", which is good but that results in "Something went wrong. If this issue persists please contact..."
