This looks like Serum was unable to load the image files for its interface. All the vector elements loaded fine, but not the images.

I'd double check you ran the right installer you can get Serums update installer, or the full installer. I'm not sure if the UI is packed in the update installer.

I'd take a moment to explore the Xfer folder in Documents, ensure all the normal content is available.
