If someone dies in PVE it is not a big suprise that you can't play as them in the main story. But for PVP they will probably never delete a hero cause it died in the lore. The game (OW1 rn) is not canon so if someone dies it does not result in they get literally deleted from the PVP game.

Yes but fighting the same groups again and again would eventually get stale. However with a variety of enemies spread across many factions, you would have to adjust your strategy on the fly. Although I'm not really talking about story here, more the Hero missions

Because he was still in a bad state - with gravitic anomalies surrounded him - and that's why he was put in isolation.

He also has implant around his eyes

But seing it from behind is really scary
